PASSPORT BOOK OR PASSPORT CARD

Passport Book
To obtain a passport as of Feb 1, 2008, you will need the following:

1. Passport application filed out completely. (Applications may be picked up in the County Clerk’s Office or printed from the website below.)
2. A certified state birth certificate or your last passport book. (Not a hospital birth certificate.)
3. Two (2) 2” x 2” passport photos. (Must be actual passport pictures.)
4. Photo ID. (Drivers License, Etc)
5. $75.00 for ages 16 and older in a money order, cashier’s check, or personal check made out to Passport Services. No cash
6. $60.00 for younger than 15 in a money order, cashier’s check, or personal check made out to Passport Services. No Cash.
7. $25.00 execution fee in cash, a money order, cashier’s check or personal check made out to Fremont County.

Passport Card
The passport card may only be used for international travel by land or sea between the Untied states, Canada, Mexico, the Caribbean, and Bermuda.

1. Passport application filled out completely. (Applications may be picked up in the County Clerk’s Office or printed from the website below.)
2. A certified state of birth certificate or your last passport. (Not a hospital birth certificate.)
3. Two (2) 2” x 2” passport photos. (Must be actual passport pictures.)
4. Photo ID. (Drivers License, Etc)
5. $20.00 for ages 16 and older in a money order, cashier’s check, or personal check made out to Passport Services. No cash
6. $10.00 for younger than 15 in a money order, cashier’s check, or personal check made out to Passport Services. No Cash
7. $25 execution fee in cash, a money order, cashier’s check or personal check made out to Fremont County.

If you need to expedite your passport book or passport card you will need the following:
1. Follow the steps above for either the Passport book or Passport card.
2. In addition to the $75.00 application fee for 16 and older, a $60.00 money order, cashier’s check, or personal check made out to Passport Services. Not cash.
3. In addition to the $60.00 application fee for 15 and younger, a $60 money order, cashier’s check, or personal check made out to Passport Services. Not cash.
